Permitting and Applications

To apply for a permit to operate an outpatient facility (permitted for sedation/anesthesia), you must complete the following:

• Apply for a New License
• Licensing checklist for outpatient facilities

Accrediting Organizations approved by the Nevada State Board of Health

• The Joint Commission
• The Institute for Medical Quality
• The Accreditation Association for Ambulatory Health Care
• The American Association for Accreditation for Ambulatory Surgery Facilities
• The Accreditation Commission for Health Care (ACHC)
