COVID-19 Vaccine Provider Verification and Pilot Pharmacy Depot Program

All partners that redistribute COVID-19 vaccine must ensure that the receiving party is enrolled in the COVID-19 Vaccine Program with the Nevada State Immunization Program (NSIP). Please review the link to verify that the receiving provider’s PIN is included on the list of currently enrolled providers. The list will be updated by NSIP weekly. If the requesting provider PIN is not included on this list, vaccine MUST NOT be transferred. Questions can be directed to DPBHCOVID19VAX@health.nv.gov .

Pharmacies that wish to un-enroll from the Pilot Pharmacy Depot Program can send an email to DPBHCOVID19VAX@health.nv.gov with a request to be removed from the list.
