Diabetes - Grants
Diabetes Prevention and Control Program is 100% federally funded by the Centers for Disease Control and Prevention (CDC) RFA DP13-1305: State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity and Associated Risk Factors and Promote School Health and receives an average of $411,463 per year to support:
- Improving the delivery and use of quality clinical and other health services aimed at preventing and managing diabetes; and
- Increasing links between community and clinical organizations to support prevention, self-management and control of diabetes.
The CDC RFA DP13-1305 funding supports statewide implementation of cross-cutting approaches to reduce the risk factors associated with childhood and adult obesity, diabetes, heart disease, and stroke.
