Healthy Behavior Support Services includes Health Coaching and evidence-based lifestyle programs delivered through community-based organizations to improve a patient’s control of hypertension and other cardiovascular disease factors. The evidence-based lifestyle programs found in Nevada are on this National Diabetes Prevention Program web page.

• Clients 40-64 years of age
• Clients with a low income (below or at 250% of the Federal Poverty Level)
• Clients who are underinsured or uninsured
