What is PATH?
The Projects for Assistance in Transition from Homelessness (PATH) grant aims to provide support to individuals with serious mental illnesses (SMI) or co-occurring substance use disorders (SUD) who are experiencing homelessness or at risk of becoming homeless.
Understanding PATH's Objectives
Objectives of the PATH Grant:
Outreach & Engagement – Identify and connect eligible individuals to essential services.
Supportive Services – Provide mental health services, substance use treatment, case management, and housing assistance.
Housing Assistance – Help individuals access stable housing and reduce homelessness.
Coordination of Care – Facilitate access to primary healthcare, employment services, and other necessary resources.
Capacity Building – Strengthen community-based systems to improve long-term support for at-risk populations.
