Licensure Application Information

If you already have an existing license and you are just making a change to your license, you can use this system to apply for any of the following:

Checklist for Initial (new facility) or CHOW Licensure Applications

For all initial/CHOW licensure applications the following items are required to be uploaded to your application:

  • Resume for the Director
    • Must list and match the director's name provided in your application.
  • Bill of Sale (CHOW only)
    • Should include information about the buyer and seller, the date of the sale, a description of the transaction, the price, and signatures from both parties.
  • Letter of Governing Body stating the effective date of change and specifying what is changing (CHOW only)
    • (i.e: change of owner from/to)
  • Copy of (Local - City or County) Business License, Conditional Use Verification Form, Zoning Approval Letter, or Special Use Permit from the local, city or county Jurisdiction.
    • Payment receipts do not meet this requirement and will not be accepted.
    • The local license must list your Doing Business As (DBA) facility name and physical location of the facility.
  • Must ensure that all of your supervisors and providers of treatment have registered with the online licensing system.
  • Must provide a position paper
    • Violence can never be condoned - NAC 228.175(1)(b)
    • Respect the plight, right, and individual differences of the victim - NAC 228.180(1-3)
    • Respect the individual differences and rights of the perpetrator - NAC 228.170(1-4)
    • Design and implement appropriate treatment programs - NAC 228.170(1-4), NAC 228.175(1-3), NAC 228.185(1-21)
    • Demonstrate ongoing evaluation of program incorporating new information - NAC 228.170(1-3), NAC 228.175(1-3), NAC 228.185(1-21)
    • Do not perpetuate sexism, racism, or victim blaming - NAC 228.110(1-10)
    • Warn victim if belief exists they are at risk - NAC 228.160(1 & 2), NAC 228.195(3)
    • Disclosure only pursuant to confidentiality laws - NAC 228.160
    • Inappropriate treatment blames/endangers victim - NAC 228.180(1-3), NAC 228.195(1-21)
    • Define when to use couple’s sessions - NAC 228.190(1)
    • Length of treatment is 6 months - NAC 228.175(2)(see also NRS dictating minimum mandatory sentence)
    • Follow ethical treatment standards - NAC 228.170(1-4), NAC 228.185(1-21)
  • Must provide a copy of the curriculum and instructional materials that will be used in the program that satisfy the requirements in NAC 228.185
  • Must submit proof director meets requirements with one of the following options:
    • a signed and dated statement from the supervisor of treatment indicating he or she will serve as the director and a copy of the director's driver’s license or other government issued ID with a date of birth on it
    • a signed and dated statement from the provider of treatment indicating he or she will serve as the director and a copy of the director's driver’s license or other government issued ID with a date of birth on it.
    • Upload a copy of the director's high school diploma or equivalent education, a resume showing the director has at least one year of supervisory or administrative experience in a related field, a copy of the director's driver’s license or other government issued ID with a date of birth on it, an attestation he or she is able to read, write, speak and understand English, and an attestation that he or she has read and understands the provisions of NAC 228.010 to NAC 228.225
  • If your program will be providing counseling sessions via electronic means, please attach a document which describes:
    • 1) How your program maintains the confidentiality of the counseling sessions so unauthorized individuals cannot overhear the sessions?
    • 2) How will you keep offender counseling records secured from unauthorize access?
    • 3) How you will meet compliance with Approved Regulations, R159-17, Section 19 (3) for offender's who reside more than 70 miles from the nearest program or for an offender which has a court order authorizing the use of online counseling sessions to attend counseling sessions or other meetings through electronic means if: 
      • (1) The offender has:(I) Attended at least one counseling session in person before attending a counseling session or meeting by electronic means and provided the program with a photograph of the offender that was taken within the immediately preceding year to be kept on file by the program; or (II) Had his or her identity verified by a court in this State before attending a counseling session or meeting by electronic means and the program provides a court with a photograph of the person who attends the counseling session or meeting by electronic means to the court to verify the identity of the person as the offender;
      • (2) The electronic means by which the offender attends the counseling session or meeting allows the provider of treatment who is overseeing the counseling session or meeting to receive audio and video of the offender; and
      • (3) The provider of treatment who is overseeing the counseling session or meeting verifies the identity of the offender by comparing the video of the offender with the photograph provided pursuant to subparagraph.
  • Must provide policies and procedures and any forms developed by the program to carry out the provisions of:
    • NAC 228.106 Payment for program: Indigent offenders; sliding scale.
    • NAC 228.115 Duties of supervisors of treatment.
    • NAC 228.117 Professional responsibility regarding offenders
