Medication Management Program Applications

Submit an application for a Medication Management Program

On this page you will find information to apply to start a new medication management program, change an existing program or renew an existing program with or without changes.

Application types:

  • Initial application to create a new program
  • Add or remove instructor to an existing program
  • Renew an existing program - NO changes (online form only)
  • Renew an existing program - WITH changes (revised curriculum must be submitted for approval along with the online form*)


Prior to submitting your application, view the information below to understand the requirements.

 

1. Submit the online application submission form.


2. Send all the required items marked with [*] below to medmgt@health.nv.gov.

 

 

Curriculum requirements

Instructor requirements (for each instructor)

  • Resume *
  • Evidence of completion of an approved 16 hour and/or 8 hours medication management class in the past twelve months *
  • Signed copy of the Ethical Obligations Agreement linked here *
  • Must have the ability to speak, read, write, and teach the entire course in English
  • Must own or have authorized use of an approved medication management curriculum
  • Must be in good standing with any applicable licensing boards (BOME, BON, BELTCA)
    • AND
  • Is a licensed healthcare professional - MD, DO, PA, NP, RN, LP – send proof of license* (if appliable)
    • OR
  • Has 3 years evidence of completion of an approved 16 hour and/or 8 hours medication management class* (if applicable)

Submit your application

Once you have all of the requirements ready to go, submit your initial, change, or renewal application for a Medication Management Course by completing both the online application submission form and then sending required items * listed above, to medmgt@health.nv.gov

 

Lastly, instructor applicants must pass the interview with the Medication Management team, in-person, or via video conference. This will occur after your required items * have been reviewed and accepted.
