Agency Permit Applications and Forms

The State of Nevada EMS Office requires all EMS agency permit applications to be submitted online using the State Online Application Portal at:https://nevada.imagetrendlicense.com/lms/public/portal#/login. 

This requirement applies to all EMS agency permit types, including ground ambulance, air ambulance, and Secured Non-Emergency Behavioral Health Transport services.

If you are a new service seeking an EMS agency permit in Nevada, please email our office at healthems@health.nv.gov, and EMS Program staff will work with your agency to establish a service account and assist with your permit application in the ImageTrend licensing system.
